Thank you for your willingness to participate in this exciting project! As you move through the questionnaire, you will be asked to answer a wide range of questions about yourself, your family, and your lifestyle. These questions were developed by scientists and doctors to help us understand how lifestyle affects our health and what influences our ability to lose weight. Your thoughtful responses will help us accomplish these goals.

To learn why we are asking for certain information, please feel free to contact us at any time by e-mail (topsgenome@wakehealth.edu) or by phone (336-713-7505). Any responses you provide will be kept confidential. Only Drs. Olivier and Cole and the Study Coordinator will have access to your personal health information.

Answering all questions will take 30-60 min. You may leave the survey at any time. If you would like to save your answers and return at a later time, scroll down to the bottom of the page and click on the “Save & Return Later” button. A screen will appear with a unique “Return Code”. It is important that you copy this code. Without it, you will NOT be able to complete the survey. You will be then be given directions for returning to your survey.

After you have completed the survey and clicked the “Submit” button you will no longer be able to access or edit your answers. Please know that your answers are very important to us and we appreciate the time you have taken to provide careful responses. Again, thank you for participating.

If you are willing to participate in this study and share your (protected) personal health information for the uses outlined in the consent form, you can proceed to the remainder of the survey.
